Abstract:	本文係描述一位慢性腎臟疾病進入腎臟病健康促進機構之護理經驗。筆者於102 年1 月21 日至102 年4 月22 日期間以直接照護、傾聽、會談、觀察、電訪、身體評估及病歷查閱等方式進行資料收集，運用Gorden 十一項健康評估，經由護理活動的介入，適時給於情緒支持及鼓勵增加信心，與營養師共同與個案指導，使個案能自行選擇適當飲食，提升控制病況及調適壓力的能力，主動參與有關慢性腎臟疾病活動。
